Students should work with their advisor and program director to submit any of the following forms:
Which RTF should I use??
RTF EZ (pdf): The RTF-EZ is used for registration transactions for the current semester for closed courses & courses requiring permission. Requires instructor or departmental/program coordinator approval. Return form to Colonial Central for processing. Time-sensitive usage (see form).
Registration Transaction Form (pdf) This form is used for special exceptions, time conflicts, grade mode changes, leave of absence/continuous enrollment, etc. Return to Program Coordinator.
CCAS Student Petition (doc)
This form requires program director approval before submission to the program coordinator (can be completed and emailed to program director).
CCAS Transfer of Credit form (doc)
This form requires program director approval before submission to the TS program coordinator (can be completed and emailed to program director). Transcript must be on file at CCAS or submitted with form for review by CCAS.
Contract for Completing an Incomplete (doc)
This form requires instructor approval before submission to the TS program coordinator. Must be completed in hard copy.
